Program Type: Community-Based

To Make Referrals: Referrals come from San Joaquin County and are prioritized based on the level of need.

# of Members: 50 members

Who We Serve:

  • Adults between the ages of 18-59 who have a serious mental illness

  • Have a history of reluctance to engage in traditional mental health treatment services

  • Have a history of repeated contact with law enforcement with multiple involuntary holds

  • The target population will comprise a combination of justice-involved individuals, including Mental Health Court and AB109 clients

Hours of Operation: Monday - Friday: 8:30 a.m. to 5:00 p.m. Staff are available on-call 24 hours a day.

About the Program: The San Joaquin Connect II program is based on the Full Services Partnership (FSP) model, where staff create a supportive environment, including wraparound services that are based on the needs, hopes, and dreams of the individuals we serve.Our staff offers advocacy for individuals who have extensive contacts with the criminal justice system. We tailor services so members can better manage conditions, monitor progress towards goals, adjust services to fit changing needs, and partner with community services to ensure continuity of care.
