Who We Serve:
BeWell clients living in Permanent Supportive Housing units in Santa Barbara County
All clients are referred to HART from Santa Barbara County BeWell
Hours of Operation: 7 days a week, 8:30 am– 5:00 pm
Warm Line Hours of Operation: 7 days a week, 5:00 pm — 8:00 am
About the Program: HART offers voluntary case management and housing stabilization services on-site at the development you live at! We will provide services seven days a week with after hours services via our warm line. Our warm line is staffed by HART peers and case managers.
The following is available to you as a participant of HART:
Services are voluntary and meet a range of needs, including social, case management, behavioral health, recovery, and housing stabilization needs.
On-site individualized supportive and case management services.
Housing stabilization services including independent living skill development, linkages to behavioral health, substance use treatment, and social services, budgeting and benefits establishment, property management and housing authority liaison support, understanding tenancy responsibilities and rights, and other identified housing needs.
ƒServices are provided throughout the community based on preference: on-site in community space; in your unit; or in the community, if appropriate.
ƒService staff will work in coordination and collaboration with you and property management to address housing stability needs.
ƒBoth individual services and group programming are available.
ƒBuilding-wide activities and events:
Social and educational/informational events will be posted and are available to any tenant.
